Product Review: AcuStratTM Helps Automate Care Management Transactions
Two years after the initial launch, AcuStrat continues to gain steam with Acuity Advanced Care® customers. AcuStrat is a user-friendly data mining and population management tool that helps automate and standardize care management processes in a wide array of healthcare settings.
According to Jean McCabe, RN, Clinical Business Analysis and Standards Specialist for Neighborhood Health Plan of Rhode Island, “After integrating AcuStrat into our care management operations, Neighborhood has improved efficiencies and quality results. By using AcuStrat, we now have an almost unlimited ability to turn patient information into actionable events, using a full array of automated functions. Among other attributes, AcuStrat can automatically:
- Stratify health and demographic data within a designated population;
- Link to various claims, pharmacy, predictive modeling and other data mining solutions;
- Schedule and assign follow-up tasks;
- Schedule mailings and turn-around time alerts; and
- Generate notifications and other correspondence.
AcuStrat also helps with quality assurance activities such as:
- QI controls for monitoring staff;
- Monitor compliance with HEDIS measures;
- Data cleanup, catching missing or erroneous information;
- Inactivity alerts; and
- Risk management programs.
McCabe elaborates, “We use AcuStrat in Neighborhood’s Bright Start pre-natal program to identify expectant mothers. The system then automatically sends these identified individuals a maternity package and, if additional triggers are met, automatically enrolls them in a wellness maternity program. We also use AcuStrat to update our Diabetes Repository based on key milestones and utilization data, and then use the TCS application to stratify the population to automatically assign health coaches to specific individuals and generate member specific mailings that include a patient report card.”
“As a result of these and other successes, AcuStrat is becoming a key strategy within our health plan to get things done and achieve many of our quality improvement objectives,” Lupoli adds. “Many of our frontline case managers now report that using AcuStrat in conjunction with ACUITY has dramatically increased the amount of time that they can spend with their patients.
